Reablement and Wellness

REABLEMENT AND WELLNESS APPROACHES: WORKING EFFECTIVELY WITH CONSUMERS

This training package was commissioned by Macarthur Disability Services and Mid Coast Communities in NSW and was developed in collaboration with Glen Sorensen from Age Communications. The project was partially funded by the Commonwealth Government through the Community Home Support Program.


There are two versions of the training package:

  • a one day workshop for case managers, service coordinators and support facilitators who work directly with consumers in the development and coordination of goal orientated support plans
  • a half to one day workshop for support workers and volunteers working directly with consumers to implement goal orientated support plans.

The workshop designed for case managers, service coordinators and support facilitators covers:

  • the foundations of reablement and wellness practice and its expected outcomes
  • the start of the process; engaging consumers in the reablement and wellness approach
  • identifying the end point; working with consumers to develop reablement and wellness goals
  • the journey from engagement to goal attainment; developing, monitoring and evaluating support plans.

The workshop designed for support workers and volunteers covers:

  • the foundations of reablement and wellness practice and its expected outcomes
  • getting support work right using reablement and wellness approaches
  • what support workers need to know about goals and support plans
  • how to best support a person to achieve their goals.
